Our Positive identification Procedures

These measures are designed to prevent errors and ensure that the cremated remains returned to the family are those of the correct individual, with the ID disc often included in the urn as proof of proper identification

CHAIN OF CUSTODY

Upon notification of someone's death, a detailed chain of custody is created.

First, this process documents the date, time, and all individuals involved in the transfer and identification.

It establishes a clear record of who has handled the said person throughout the entire procedure.

IDENTIFICATION BRACELET

Upon the transfer technician's arrival, a legally authorized party releases the said decedent, signs a transfer document verifying the identity and such release.

At that point an ID bracelet is securely affixed to the decedent. This bracelet contains information such as the name, date of birth, date of death and the name of our facility: Florida Direct Cremation.

LOGIN & DOCUMENTATION

The transfer technician escorts the decedent directly to our crematorium including delivery of the transfer documents and belongings.

In addition, the tech completes a receipt of release form and enters the decedents name on the login sheet.

Next, assigning an internal unique identification number with a consecutive corresponding steel ID Tag.

The decedent is placed into an Alternative Container (Cardboard Coffin) designed specifically for cremation and their ID Tag is secured to the outside of that container with the receipt of release.

SECURE ON-SITE STORAGE

Your loved one sheltered in a climate-controlled area within our crematorium.

Then, the crematory staff compares the information on the transfer documents and login sheet with the ID bracelet and steel ID Tag.

This step ensures the accuracy of the decedents information and is noted with the licensed person in charge.

VISUAL EXAMINATION

Our cremation professionals respectfully perform a thorough visual examination of the decedent.

Checking for pacemakers and/or other bailor property noting any unique physical characteristics, scars, birthmarks, tattoos or other identifiable features that can assist in a positive identification.

CREMATION AUTHORIZATION REVIEW

The crematory staff carefully reviews the cremation authorization form, ensuring it has been properly completed and signed by the appropriate individual(s) with the legal authority to authorize the cremation.

POSITIVE ID CHECK & FINGERPRINTING

Before the cremation takes place, a final verification is performed by a designated staff Member. They cross-reference the identification tag, paperwork, and other relevant documentation to confirm that all details match and that no discrepancies are present.

Finally, fingerprints are captured using a biometrics device and are provided to the family for safekeeping.
